1. INTRODUCTION

This manual is published in accordance with the requirements of the Promotion of Access to Information Act, 2 of 2000 (PAIA), as amended by the Protection of Personal Information Act, 4 of 2013 (POPIA).

Dr. Priya Parbhoo is a registered specialist Cardiologist in independent private practice at Netcare Blaauwberg Hospital, Sunningdale, Cape Town.

This manual provides information about the types of records held and the procedures for requesting access to such records.

5. REQUEST PROCEDURE UNDER PAIA

Requests for access to records must:

Be made in writing on the prescribed Form C (available at justice.gov.za or upon request).

Provide sufficient detail to identify the record and the requester.

State the requester’s identity and contact details.

Indicate the form of access required and preferred language (if applicable).

Include proof of payment of the request fee (if applicable).

6. GROUNDS FOR REFUSAL OF ACCESS

Access may be refused if:

The record contains personal or medical information about a third party.

Disclosure would breach confidentiality, medical ethics, or doctor-patient privilege.

It would endanger the life or safety of an individual.

It is protected by legal privilege or relates to ongoing legal proceedings.

 

7. FEES

Fees may be charged as permitted by PAIA:

A request fee (payable before processing the request).

An access fee (based on time required to search, prepare, and provide the record).

Fee schedules are available from the Information Regulator or on request.
